FEDERAL-STATE INCOME TAX WITHHOLDING (W-4 & DE-4 Forms) - PDF File
Complete Form W-4 so that Stanford can withhold the correct Federal income tax from your pay. Because your tax situation may change, you may want to refigure your withholding each year.

DIRECT DEPOSIT FORM - PDF File
Have your paychecks directly deposited to your financial accounts.

EMPLOYMENT ELIGIBILITY VERIFICATION (I-9 FORM) - PDF File
All Stanford employees must have an active I-9 form on file. This form requires copies of your passport, and if you are an Alien, your I-94 entry card, Visa, DS-2019, etc. Read the enclosed instructions for more information.

SHELL FUND APPLICATION FORM - PDF File
The Shell Foundation provides funds for students in the School of Earth Sciences who travel in relation to the academic work they are doing. This can be to deliver a research paper to a professional meeting or to conduct field work related to the research project you are doing to earn your degree. Funds are generally limited to one time per student per academic year.
